Preventive Health Care means assessing risk for health conditions, and preventing, delaying, or managing those conditions starting at a young age.
All Community Care Facilities must ensure that each person has access to all needed medical and dental services and that their health care needs are met. Each person’s Individualized Program Plan (IPP) should specify how frequently preventive routine physical examinations and other routine health care are to be maintained.
In responding to a person’s health care needs, you must often find resources to meet those needs.
These basic resources are:
A primary care doctor (or group)
A dentist who does family or general dentistry
Specialists (Example: an eye doctor, gynecologist, podiatrist)
Regional center clinicians
Support group for people struggling with kidney disease
Information sources:
Self-care handbooks; voluntary organizations like the American Cancer Society or the American Heart Association;
Internet resources
